Release of Liability
I/we (the dancer and parent/guardian) realize that participation in the competition Dance Prix de New York, dance classes and performances, and related and unrelated activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we assume all risks related to the participation in the dance programs and use of any and all spaces in which such events may occur.
I/we agree to release and hold harmless the Dance Prix de New York, its producers, organizers, staff, volunteers and affiliates, the University hosting said events and any teachers used from any cause of action, claims, or demands now and in the future. I/we will not hold the aforementioned liable for any personal injury or any personal property damage, which may occur on the premises before, during or after the events.
I/we understand that understand that touch is part of an instruction process and hold harmless the aforementioned producers, organizers, teachers and studio for teacher's light touching of appropriate body parts as a necessary part of dance instruction.
I/we agree to allow the Dance Prix de New York and its affiliates to photograph and/or video tape any portion of the events and to alter, distribute or dispose of any and all media in any means including use in promotional, educational and any and all other purposes as should be desired.
By submitting this form, I/we agree to these terms for the duration of my/our participation in the Dance Prix de New York.
